SHAR Inc. is implementing an Outcomes Management System (OMS) to ensures effective patient care. The SHAR Inc. OMS is derived from the Kellogg Foundation Logic Model Development Guide. More information on this guide can be found here. Creating a logic model ensure the efficacy of the program while embedding a flexibility that allows the program to change without losing focus on its core mission.
The purpose of a logic model is to provide stakeholders with a road map describing the sequence of related events connecting the need for the planned program with the program's desired results. Mapping a proposed program helps stakeholders visualize and understand how human and financial investments can contribute to achieving the intended program goals and can lead to program improvements. Within the context of a logic model stakeholders are able to try an idea on for size and apply theories to a model or picture of how the program would function.
Using a logic model will allow SHAR Inc. to gather information that will then influence decisions about resources. These decisions will allow SHAR Inc. to meet its stated goals. Applying the process consistently positions SHAR Inc. for success by laying out the best course of action and providing benchmarks for measuring progress.
The SHAR Inc. Outcomes Management System (OMS) will point out areas of strength and weakness. OMS will allow stakeholders to run through many possible scenarios to determine the best possible outcome. OMS will make it possible to adjust approaches and change course as the program plans are developed. Ongoing assessment, review, and corrections will produce a better program design. An additional benefit to OMS is that it will create a system to strategically monitor, manage, and report program outcomes throughout all phases of development and implementation.
