Project Summary
In collaboration with the Office of Faith-Based and Neighborhood Partnerships (OFNP) - Clergy Leadership Advisory Council, a major multi-denominational faith-based entity, SHAR has developed an innovative communityplan called Enhancing Community Health Outreach (ECHO) to address the escalating social problems within the Greater Detroit area.
ü ECHO is an early intervention, recovery-oriented, faith-based outreachprogram designed to help residents and their families, by providing a wide range of community-based services (triage, prevention, education, training, treatment and referrals) based on individual, family and community need.
ü Through faith-based organizations, ECHO will establish a multi-disciplinary recovery-oriented continuum of services and coordination of care that promotes individual recovery along with family and community health and wellness – and restore functioning.
ECHO is a Therapeutic Community without walls delivered in reliable & trusted settings.
